This Compilation bundles all SEVEN Leadership Health Evaluation Forms and Assessments in one package (7 separate files). It contains the following Assessments and Tools:

Character Assessment (Leaders)
This is a 25 question survey based upon the character qualities for leaders found in Timothy and Titus. Each question is valued on a scale of 1-4 to help evaluate the mentoree’s commitment to character development. It is also included in Mentoring thru Intentional Relationships Guide 1 and the Leadership Foundations Workshop.

Motivation and Gifts Assessment
This is a ten page assessment tool to help someone understand their basic gifting and motivation and possible roles that this may indicate. The questions guide the mentoree through an Inward Conviction Questionnaire, how Gifts are related to Traits and Confirmation of Spiritual Gifts. Each of these are designed to be answered and then discussed with a mentor. It is also included in Mentoring thru Intentional Relationships Guide 1 and the Leadership Foundations Workshop.

Mentoring Intelligence Assessment (Leadership MQ)
This 25 question assessment can be used as an indicator of one’s capacity to be a mentor. Each of the questions reflects a leadership and mentoring principle necessary to be an effective mentor. As with EQ (Emotional Intelligence) and CQ (Cultural Intelligence), MQ is a learned skill and this resource is helpful to indicate the areas of growth needed. It is also included in Mentoring thru Intentional Relationships Guide 1 and Guide 2, Coaching Guide and the Mentoring Intelligence textbook.

Personal Calling and Role Survey
While vision is the seed, values are the soil in which it grows. Values must be able to sustain the vision and values are expressed in behaviours. These series of questions help to identify a person’s core values and their personal life vision and from there help to determine one’s kingdom role or personal calling statement. This resource is also included in Mentoring thru Intentional Relationships Guide 1, and the Leadership Foundations workshop.

Personal TimeLine
This is a simple one page template that can be used to begin a personal Timeline. It also comes with a one page instructional sheet to help in filling out one’s own Timeline. This is also included in Mentoring thru Intentional Relationships Guide 1, and the Leadership Foundations workshop.

Relational Health Assessment (Leadership)
This is a 36 question assessment that can be used to evaluate the emotional health of a leader. Each of the questions can be discussed with a mentor to gain further insight into areas of needed growth. The questions address areas of values when to comes to leadership and dealing with others. It is also included in the Mentoring thru Intentional Relationships Guide 2.

Servant Leadership Assessment
This 30 questions assessment highlights the key principles of servant leadership. It can be used by individuals and teams to address what areas of growth of needed to better reflect the type of leadership that Jesus exemplified. This is also a great discussion tool to be used within a mentoring relationship. While this resource will not make someone a servant leader, it can highlight one’s values and areas of realignment and growth. It is also included in Mentoring thru Intentional Relationships Guide 2.
