ORGANIZATIONAL HEALTH RESOURCES (OHR)

As with the Leadership Health Resources, these Team resources are provided in our various Guides, manuals and workshops. They are also provided as stand-alone tools, although it is recommended that they be used together with the corresponding workshop or teaching material. This Compilation bundles all SEVEN Organizational Health evaluation forms and assessments in one package (7 separate files, 8 in English). It contains the following Assessments and Tools:

Mentoring Intelligence Assessment (Organization)
These 25 questions can be used with individuals and a team to assess how “friendly” the organization, church, or ministry team is to mentoring types of relationships. Each question reflects a principle of developing mentoring friendly structures as discussed in both the Introductions of the Mentoring thru Intentional Relationships and Developing Others workshops. This assessment is also included in the Mentoring thru Intentional Relationships Guide 1 and 2, Coaching Guide and Mentoring Intelligence textbook.

Relational Health Assessment (Organization)
This is a 36 question assessment that can be used to evaluate the emotional health of a ministry team. Each of the questions can be then discussed together as a team and with a ministry coach to determine any blind spots in the organizations with regards to the health of the relationships and steps needed as a team to move forward. It is also included in the Mentoring thru Intentional Relationships Guide 2.

Team Building Checklist
This is a one page simple team building checklist to help a leader determine how effective they are as a team leader. Through a series of questions, a team and team leader can receive a snapshot of how they are doing in relation to the building a team. This is also included in the Mentoring thru Intentional Relationships Guide 2.

Vision Assessment (only available in English, Russian, Spanish)
This four page questionnaire includes both a 30 questions evaluation component, as well as ten open ended questions that are intended to lead to discussion on a team. Together they are intended to help the team leader and team members identify, process and evaluate their vision. Vision is crucial in maintaining the health of a ministry or organization. Organizations are never stagnant, they are either moving forward or backwards. This tool will help you change, simplify, clarify, or perhaps even abandon parts of your vision.

Ministry Flow Chart
This six column chart can be used as a planning tool for new ministries or evaluation tool for existing ministries. The Ministry Flow Chart is a diagram that displays ministries, leadership gifts and leadership functions and how they work together in areas of pre-evangelism, evangelism, connecting growth and maturity, leadership development and church multiplication. This resource includes a description of each of the categories of the Flow Chart as well as an explanation as to how it is used with a group. It is also included in Mentoring thru Intentional Relationships Guide 2, the Coaching Guide, and the Team Building workshop.

Worship Song Evaluation (only available in English Version)
This evaluation begins with 25 questions with a 1-4 scale, addressing issues as to the technical aspects about a song as well as the heart and theological issues. While the attributes of a song are somewhat abstract, this evaluation seeks to measure and create discussion regarding specific outcomes desired by a good worship song. This can be used by the songwriter personally, or by worship teams as they determine which songs have merit to be used within the worship service. The second part of the evaluation organizes all 25 questions into five categories: Title, Message, Melody, Chords, and Impact. It is also included in the English version of the workshop, Worship Teams.

CHEA – Church Health and Effectiveness Assessment
This 75 question survey is designed to assess the health of a church and its ministries. The purpose of this assessment is to determine short, medium, and long term goals that will help point the church toward reaching its potential. It will assist leaders learn about the current state of their ministry so that a measurable plan can be implemented. It can be administered as a small group synthesis and summary or a comparative data analysis. The questions evaluate fifteen categories that include the six categories of the Flow Chart and the four categories of the Life Cycle. Notes of explanation are also included for both the Flow Chart and Life Cycle to help in the explanation of the results. This tool is also included in the Coaching Guide and it is recommended that this assessment be administered together with that Guide or together with one of our Coaching team.

CPLC – Church Planters & Leaders Competency Assessment
This is a 120 questions, ten category competency assessment designed to determine an individual leaders attributes to be involved in a church plant or ministry start up. While no one leader possess all of these, team leaders need to be strong in several key area, as these have shown to be indicators of ministry effectiveness. This tool can also be used to determine needed team members for a church plant according to giftings and function. This is best used in conjunction with a mentor who can help guide the mentoree in developing themselves and identify the type of team members necessary to be effective. It is also included in the Coaching Guide.
